Here at Dr. Files' office, we understand how busy you are and value both you and your time. This is why we now make our new patient forms available for download, providing you with the option of completing each at your leisure and in the comfort of your own home, or after arriving in our Redmond WA office.

Should you opt to complete these forms at home, simply print them out, fill in your information, and bring in for your first visit. If you complete them at our office before seeing Dr. Files, you may want to arrive a few minutes early to allow yourself time to compete them without feeling rushed, for that’s the last thing we want!
